House rules Plexus Amsterdam

1. Reimbursement – If you have supplementary health insurance, then physiotherapy and manual therapy are (partiall) reimbursed. Before your first visit, check your policy conditions to see whether you are insured for treatment and for how many sessions. Only in the case of a chronic indication (according to the ‘chronic list’) is physiotherapy reimbursed by the basic health insurance, from the 21st session onwards. Plexus Amsterdam is not responsible for the reimbursement.

2. Payment for non-insured care – Treatments that are not reimbursed by your health insurer will be charged to you directly.

3. Registration – When you register, you will be asked for your contact details (name, address and telephone number), your insurance details and - if applicable - the referral note from your doctor or specialist. We are obliged to include your Citizen Service Number (BSN) in our administration.
4. First appointment: intake and assessment – Your first appointment consists of an intake and assessment. During this appointment, we will discuss your complaints, medical history, and goals, followed by a physical examination.
   
   Based on this information, we will create a treatment plan together with you, outlining the treatment goals, methods, and the expected number of sessions.
   
   As this appointment focuses on diagnostics and treatment planning, there may not always be sufficient time to start a full treatment immediately. A correct diagnosis and treatment plan are necessary to provide appropriate and responsible care. If needed, the first treatment will take place during a follow-up appointment.
5. Valid ID – Please bring a valid form of identification (ID card, driver’s license, or passport) and your health insurance card to your first appointment.
6. Cancelling appointments – If you are unable to attend, you must cancel your appointment at least 24 hours in advance. Otherwise, we are obliged to charge you for the treatment.
7. Parking – Please park your car, bicycle, or other vehicle so that passageways remain clear and others can move freely.
8. Conduct – We expect patients to treat staff and other patients respectfully. Unacceptable behavior, aggression, or intimidation will not be tolerated. Plexus Amsterdam appreciates it when patients are considerate of each other and, where possible, leave seats available for less mobile patients.
9. Valuables – Do not leave any valuables in the waiting room (e.g. on the coat stand) or treatment room. Plexus Amsterdam is not liable for loss or theft of personal belongings.
10. Waiting room literature – If you use the reading material provided, please return it after use.
11. No smoking or eating – Smoking and eating are not allowed in the practice.
12. Emergencies – In case of emergencies, always follow the instructions of the staff or authorized authorities.
13. Hygiene – When training, please bring your own towel. Wear comfortable sports clothing and clean sports shoes.
    We expect patients to observe the hygiene rules and ensure good personal hygiene during their treatment.
14. Pets – It is not permitted to bring pets into the practice, with the exception of animals needed to assist with a handicap.
15. Complaints procedure – If you are not satisfied with your treatment or our services, please report this to one of the partners of Plexus Amsterdam. Your complaint will be dealt with in accordance with the guidelines of the KNGF.
16. Agreement to house rules – By making an appointment at Plexus Amsterdam, you agree to these house rules.
